Telehealth Services Addendum

Effective Date: March 1, 2026

This Telehealth Services Addendum (“Addendum”) supplements the Terms of Use for BlueHaven Wellness and governs the provision of telehealth services. By engaging in telehealth services, you acknowledge and agree to the following terms.

1. Nature of Telehealth Services

Telehealth services are provided through secure, electronic communication platforms that allow remote interaction between clinician and client.

Telehealth may include:

    • Video conferencing
    • Secure messaging
    • Electronic document exchange

Telehealth services are considered equivalent to in-person services in terms of professional standards of care.

2. Eligibility & Location Requirements

Telehealth services are provided only to individuals physically located in states where the clinician is licensed and legally authorized to practice at the time of the session.

Clients are responsible for:

    • Providing accurate location information at each session
    • Informing BlueHaven Wellness of any change in physical location prior to services

Services may be discontinued if jurisdictional requirements are not met.

3. Risks of Telehealth

While reasonable safeguards are implemented, telehealth services may involve potential risks, including but not limited to:

    • Technical disruptions (internet failure, platform malfunction)
    • Unauthorized access despite security measures
    • Limitations in assessing non-verbal cues compared to in-person services

By participating in telehealth, you acknowledge and accept these potential risks.

4. Technology Requirements

Clients are responsible for ensuring they have:

    • Reliable internet access
    • A private and secure location for sessions
    • A device capable of supporting secure video conferencing

BlueHaven Wellness is not responsible for technical difficulties arising from the client’s equipment or internet connection.

5. Confidentiality

Telehealth sessions are conducted using HIPAA-compliant platforms when applicable. Clients are responsible for maintaining privacy on their end by choosing a secure and confidential environment.

Sessions may not be recorded by the client without prior written consent.

6. Emergency & Crisis Limitations

Telehealth services are not appropriate for emergency or crisis situations.

BlueHaven Wellness does not provide 24-hour crisis coverage. If you are experiencing a mental health emergency, you agree to:

    • Call 911
    • Contact 988 (Suicide & Crisis Lifeline)
    • Go to the nearest emergency room

Clients may be required to provide an emergency contact and local emergency resources prior to initiating telehealth services.

7. Payment & Cancellation

Telehealth sessions are subject to the same scheduling, cancellation, and payment policies as in-person services. Additional details are provided in the Informed Consent and Financial Policy documents.

8. Termination of Telehealth Services

BlueHaven Wellness reserves the right to determine whether telehealth services remain clinically appropriate. Services may be transitioned to in-person referral or terminated if:

    • Telehealth is no longer clinically suitable
    • Safety concerns arise
    • Jurisdictional limitations apply

9. Acknowledgment

By participating in telehealth services, you acknowledge that you have read, understand, and agree to this Telehealth Services Addendum in addition to the general Terms of Use.
